Needs Assessment
The Care Manager will ask a variety of questions to identify where your loved one might need support, as well as to understand which parts of their daily life are currently going well.
Review of ADLs
The Care Manager will then ask about common Activities of Daily Living—such as bathing, dressing, cooking, and moving in and out of a chair or bed—to determine whether these tasks are still manageable or if assistance is needed.
Identify Opportunities for Support
After evaluating the situation, the Care Manager will discuss care options that may be helpful for the family, based on any needs or gaps that were identified.
Home Safety Check
A home tour will help uncover any potential safety issues in the environment, such as tripping hazards, missing grab bars in wet areas, improper use of extension cords or space heaters, or other safety concerns.
Social Assessment
Next, the Care Manager will assess any physical or emotional risks that may arise from isolation and loneliness.
Care Coordination Plan
The Care Manager will then review any health concerns, medication management needs, and other important issues that should be included in a coordinated care plan.
Care Consult Recommendations
We’ll go over the results with you and offer personalized recommendations. Together, we’ll discuss the type and amount of care that would best meet your family’s needs.
